Required Documents

Join us May 17 from 10am-1pm for an in-person registration, form drop off, meet the staff (some of them), and tours of the campus at Camp Coch, 291 Beach Street.

Health Form 

Every camper must have an up-to-date health form on file to attend camp. 

Health form must be:

• Top portion signed by parent
• Signed by a health practitioner
• Submitted to camp by June 20th
• Be within 3 years of last exam

Medication/Screen

• If your camper has medication they need to self-administer, please have your camper's doctor fill out the Authorization for the Administration of Medication form. 
• Sunscreen cannot be applied by staff.  Please fill out the sunscreen self-administration form if you would like your camper to be able to apply sunscreen throughout the day. Please apply the initial layer before drop off at camp. 

Pickup Form

• Please include the individuals who may be picking your child up from camp - please inform Director or Assistant Director or Counselor at drop off who will be picking up each day (a written note is most helpful)
