Five Wishes: An easy approach to advance care planning

Join us for an exploration and discussion on advance care planning. Create a plan for yourself you can share with those who love and care for you. Making choices for your healthcare at the end of one's life can be an empowering experience and a gift to one's family.

Catherine Vlasto is a Goshen resident and works as a licensed clinical social worker who has experience in end-of-life issues and is currently in private practice.

This is a hybrid (in-person and Zoom) program.
