Dog Licensing is upon us!

 

 

Please come in or mail your dog information. We will need proof of rabies and if applicable spaying/neutering papers.

All dogs six month of age or over must be registered. Licensing starts June 1 – June 30. If you are registering your dog for the first time in Goshen please remember to bring in a current rabies certificate and papers showing that your dog is spayed or neutered. You may also submit this by mail as well. The cost is as follows:

 

  $8.00 for a spayed or neutered dog

$19.00 for an unaltered dog

$51.00 for a kennel license

  $1.00 per month late fee

-Owners Name and Address

-Proof of Rabies

-Proof of Spaying/Neutering if not gender of dog

-Age of Dog

-Phone Number

-Breed of Dog

-Color of Dog

-Dogs Name

                        e-mail address

 

We are able to put in a chip number and an e-mail address so that in the future to save on postage we will be able to e-mail you a renewal reminder.
